Why do the same team challenges keep repeating?

Updated July 30, 2026

Recurring team challenges are often a sign that the team is responding to new situations with old patterns.

A team may continue revisiting decisions, avoiding conflict, struggling with accountability or experiencing the same tensions despite multiple attempts to resolve them. While the presenting issue changes, the underlying dynamic often remains the same.

Many of these patterns originate in earlier experiences. They may have been shaped by a previous leader, a period of organizational uncertainty or a way of working that was once highly successful.

Until those patterns are understood, teams tend to recreate them, even when they no longer serve the team or the organization.

Lasting change requires more than solving the immediate problem. OSC uses the Origins Map™ to help leadership teams understand the assumptions and dynamics that continue to reproduce it.
